Patient Pathways for Diabetic Patients
Sia Partners oversaw a project for a local actor who wished to implement patient pathways for diabetic patients. We intervened in all stages of the project, from helping to identify their needs to the full deployment of the patient journey (a tool to coordinate medical care, educational therapy, coordinating stakeholders etc).
